Why Your Vitamin D Status In Midlife Could Shape Your Cognitive Future

Your 30s and 40s are when you build the body you’ll live in decades from now. That applies to muscle, bone, metabolic health — and, increasingly, your brain.

New research suggests your vitamin D status in midlife may influence how your brain ages decades later. The key finding? Adults with higher vitamin D levels in their 30s and 40s had significantly less tau burden on brain scans roughly 16 years later, particularly in the brain regions affected early on in the progression of Alzheimer’s disease. That’s notable because most vitamin D research focuses on memory, cognitive performance, or dementia diagnoses — and less often on biological changes inside the brain itself.

It’s not proof that vitamin D prevents dementia. But it’s another piece of a growing puzzle suggesting brain health may depend on your everyday habits long before memory problems ever appear. Inside the Study

Published this April in Neurology Open Access, the study recruited 793 dementia-free adults with an average age of 39. Researchers measured their vitamin D levels, then used PET scans roughly 16 years later to assess tau and amyloid accumulation. The result: Higher vitamin D levels were linked to less tau, but not less amyloid.

To understand why this matters, it helps to understand the two hallmark proteins involved in Alzheimer’s disease: amyloid and tau. Amyloid builds up outside brain cells, forming sticky plaques that are thought to kick off the disease process, sometimes years or even decades before symptoms appear. Tau, meanwhile, builds up inside brain cells, forming tangles that disrupt the cells’ internal transport system and damaging the neurons therein. 

Both proteins matter. But tau tends to be a better predictor of whether cognitive problems will arise.

“Tau generally correlates more closely than amyloid does with brain atrophy and cognitive symptoms,” says David Perlmutter, MD, a board-certified neurologist, six-time New York Times bestselling author, and the author of Brain Defenders: Harness the Power of Your Immune Cells to Protect Your Brain for Life. “The study therefore suggests that vitamin D status may influence a pathway that is particularly relevant to how Alzheimer’s ultimately affects brain function.”

What This Does and Doesn’t Mean

Before you stock up on vitamin D supplements and think you have your brain bases covered, know that this study has its limitations.

First, this was an observational study. It found an association, not solid proof vitamin D prevents tau accumulation or Alzheimer’s disease. A healthy-user effect could also be at play. “Higher vitamin D may partly reflect other healthy behaviors, including outdoor activity, exercise, diet, and metabolic health,” says Dr. Perlmutter. The researchers adjusted for relevant factors such as age, sex, mental health, season, and medications, but they couldn’t fully account for the aforementioned lifestyle considerations. 

Plus, Dr. Perlmutter points out that vitamin D was only measured once (levels can change substantially over the years) and the subject pool was somewhat narrow: “The sample was predominantly White, limiting confidence that the findings apply equally across racial and ethnic populations,” he adds.

There’s another key reason to keep your expectations in check. Alzheimer’s isn’t driven by tau alone. How The Findings Fit Into Existing Research

The new findings don’t exist in a vacuum. Per a 2023 meta-analysis of 24 randomized trials published in Neuropsychology Review, vitamin D supplementation significantly influenced global cognition, with a greater effect in vulnerable populations and those with a vitamin D deficiency at baseline. And a 2019 meta-analysis of 16 studies in BMC Neurology found that participants with vitamin D levels between 10 and 20 ng/mL had a 36% higher Alzheimer’s risk, plus a 50% increase with levels below 10 ng/mL. 

“Unfortunately, levels like these are not uncommon in American adults,” Dr. Perlmutter warns. The Cleveland Clinic estimates over a third of adults in the United States have a deficiency and half of the global population has an insufficiency.

So how low is too low, and where’s the sweet spot? The National Institutes of Health considers levels below 20 ng/mL deficient. Levels from 20-29 ng/mL are generally considered insufficient. And 30 ng/mL is generally considered sufficient. 

The newer Neurology Open Access study “does not establish that pushing levels higher and higher is beneficial,” says Dr. Perlmutter, “nor does it prove that the exploratory 55-to-99 ng/mL range should become a treatment target.” 

But together, the evidence suggests that maintaining adequate vitamin D levels may support long-term brain health. It also emphasizes that correcting a deficiency earlier could have a meaningful impact years down the road.

The Best Way to Boost Your Levels of the Sunshine Vitamin

Three things influence your vitamin D status: sunlight, food, and supplements.

“Sunlight is the most natural source and also supports circadian biology, but it’s not always reliable,” says Dr. Perlmutter. “Latitude, season, age, sunscreen, clothing and skin pigmentation all affect vitamin D production.” 

Of course, there’s also the risk of skin cancer to contend with. (Read: Don’t embrace tanning for the sake of raising your levels.)

Then there’s your diet. By and large, it’s best to get your nutrients through the foods you eat first. But vitamin D might be a rare exception. It’s one of the few nutrients that’s genuinely difficult to get in adequate amounts from food alone with sources limited to the likes of fatty fish, egg yolks, fortified milk, and cereal.

For many people, especially those who are deficient, that leaves supplementation. There’s just one caveat: Your dose should be supported by labs, not guesswork. 

“I begin by measuring serum 25-hydroxyvitamin D rather than supplementing blindly,” says Dr. Perlmutter. “The appropriate dose depends on the starting level, body weight, sun exposure, skin pigmentation, absorption, kidney function, and medications.” Once your healthcare provider considers these factors, they can determine your appropriate dose. You’ll also want to re-check your levels to make sure they remain in a healthy range. “Vitamin D is fat-soluble, and excessive supplementation may cause hypercalcemia, kidney stones, and other complications,” Dr. Perlmutter warns.

The Takeaway

The biggest takeaway isn’t that everyone should start taking vitamin D today. It’s that brain aging begins long before memory slips do. Checking your vitamin D level is relatively simple. Correcting a deficiency is straightforward, too. And while it won’t guarantee protection from dementia, it’s one more reminder that the choices you make in midlife help shape the brain you’ll rely on decades from now.

“The larger message is not that vitamin D is a cure or fully preventative as it relates to cognition,” says Dr. Perlmutter. “It is that a relatively simple, measurable, and potentially modifiable factor may influence tau biology decades before dementia develops. That is precisely when prevention matters most.”

Want to take even further precautions to keep your brain healthy? Dr. Perlmutter suggests testing your levels of vitamin B12 (“arguably the most important deficiency not to miss”), folate, vitamin B6, magnesium, and omega-3. A recent study in older adults in Japan also found that higher vitamin C levels were linked to healthier brain structure and stronger brain network connections involved in memory, attention, and other cognitive functions.

Even still, nutrient status remains one part of the picture. Certain biomarkers — like fasting glucose, insulin or hemoglobin A1c, lipids, blood pressure, inflammatory markers, and thyroid function — may be even more influential than vitamin D, says Dr. Perlmutter.

“Ultimately, nutrient optimization works best as part of a larger program that includes regular aerobic and resistance exercise, restorative sleep, metabolic control, a nutrient-dense diet, stress management, and meaningful social connection,” he says. In other words, focusing on the pillars of a healthy lifestyle is likely to keep your brain in fighting shape as the years go by. Vitamin D can help, especially if you’re one of the many with suboptimal levels, but it’s a team effort that’ll really move the needle for your brain and body alike.
